TRANS-SENIOR fellows presented their work at the GSA 2021

On the 11  ^th of November 2021, some of the TRANS-SENIOR fellows presented their work at theGSA 2021 Annual Scientific Meeting. Unfortunately, two months before the start of the conference, it was announced that the conference would be organized in an all-online format. Nevertheless, it was a very interesting conference with as a central theme “Disruption to Transformation: Aging in the “New Normal”. Four ESRs had the opportunity to present during the symposium titled “Transitions to Long-Term Residential Care Settings”, during which we zoomed in on the three work packages of the TRANS-SENIOR program; Ines Mouchaers presented the development of a reablement program to prevent unnecessary care transitions after geriatric rehabilitation. Lindsay Groenvynckfollowed to discuss the mismatch between theory and practice in a transition from home to a nursing home, where she presented the results of her recently published scoping review. Next,Megan Davies gave insight into resident experiences on transition into long-term residential care during the COVID-19 pandemic. Amal Fakhaclosed this symposium, discussing the implementation of transitional care innovations, with the main message considering organizational context and process as key. To conclude, the discussant Prof. dr. dr.
