Developing guidance for necessary & avoidable care transitions in senior citizens

Objective of the project:
This research project aims to develop evidence and consensus-based guidance on necessary versus avoidable care transitions in senior citizens. It will focus on 1) a systematic review of evidence on avoidable transitions and 2) develop consensus among senior citizens, informal caregivers, professionals and care organisations regarding characteristics of necessary versus avoidable care transitions in senior citizens (using selected cases from the work of other early stage researchers in the TRANS-SENIOR consortium). Furthermore two practice tools that support the decision making of a) senior citizens and informal caregivers and b) care professionals will be developed and pilot tested.
This project will provide practical insights on how to discriminate necessary from avoidable transitions and indicate where local differences influence the recommendations.
