Developing international guidance for optimal transitional care in senior citizens


Project update:

Objective of the project:
To develop guidance for essential elements of good transitional care for senior citizens.

Expected results:

  • ESR 6 will deliver guidance and practical tools for supporting optimal transitions among senior citizens. Tools will be developed to support a) senior citizens and their informal caregivers and b) care professionals.
  • Further results from the work performed by ESR6 are an overview of the evidence for methods to facilitate the improvement of care transitions, and consensus on relevant elements of support for improving these transitions.

These products will have high generic value and will be developed for international use.
